Privacy Statement

The Department of Education (DoE) is collecting the personal information requested in this form in order to obtain lawful consent to disclose your child's personal information to FOGS At Work Limited for the purposes of the FOGS ARTIE Academy Program (Program) and the Department of Aboriginal and Torres Strait Islander Partnerships (DATSIP). FOGS At Work Limited is collecting the personal information requested in this form in order to obtain lawful consent to disclose your child's personal information to DoE for the purposes of the Program. The information collected on this form will only be accessed by authorised DoE staff and will be dealt with in accordance with the confidentiality requirements of s.426 of the Education (General Provisions) Act 2006 (Qld) and the Information Privacy Act 2009 (Qld). The information wil not otherwise be disclosed to any other person or agency unless it is for a purpose stated above, the disclosure is authorised or required by law, or you have given DoE and FOGS At Work Limited permission for the information to be disclosed. If you have any concerns about the use of your personal information contained on this form please contact the school in the first instance.

Section 1: Information Sharing

This consent form records your permission for the disclosure of student personal information by:

a) the Department of Education (DoE) to FOGS At Work Limited and the Department of Aboriginal and Torres Strait Islander Partnerships (DATSIP); and

b) FOGS At Work Limited to the DoE, noting that the DoE has no control over the information once it is disclosed, FOGS At Work Limited, and that the information is disclosed entirely at your own risk.

By signing this form you give permission for the following personal information, about the student nominated below to be disclosed to FOGS At Work Limited:

  • Name, date of birth, contact details and cultural identification (Aboriginal and Torres Strait Islander);

  • Individual attendance and absence information including school disciplinary absences and negative behavioural

  • Incidences (on a weekly or other regular basis):

  • School report card (on a Term and semester basis);

  • NAPLAN results;

  • ID Photograph (once only); and

  • Class list and/or timetables (dependent on when changes occur).

FOGS At Work Limited is collecting personal information on this form for the purposes of:

  • Determining your child's eligibility for participation in the Program;

  • Meeting its reporting obligations to the State of Queensland (acting through the DoE and DATSIP)

  • Planning, administering and managing the delivery of the Program;

  • Maintaining the good order and management of the Program and to fulfil its duty of care to children and staff;

  • Evaluating and reviewing the Program and its outcomes for participants; and

  • Communicating with children and parents/guardians.

Personal information disclosed to the DoE and DATSIP will be used by the DoE and DATSIP for the purposes of:

  • Facilitating you child's participation in the Program;

  • Evaluating the Program and performance of FOGS At Work Limited, and planning future service delivery; and

  • Communicating with parents/guardians to support future educational plans and promotion of education options.

This consent is valid only for the school year in which it is given, unless withdrawn earlier.

Section 2: The FOGS ARTIE Academy Program

FOGS At Work Limited assists to 'Close the Gap' in educational outcomes between Aboriginal and Torres Strait Islander students and their non-Indigenous peers. It provides Aboriginal and Torres Strait Islander students attending Queensland State Schools the opportunity to participate in a program that encourages and rewards improvements in school attendance, behaviour, academic achievement, effort and ultimately Year 12 attainment or its equivalent.

The Program is provided by FOGS At Work Limited, ACN 117 675 877, an Australian Public Company limited by shares (https://www.artie.net.au).

Please Note: FOGS At Work Limited and the Program is not part of the DoE and the DoE has no control over the operations of FOGS At Work Limited FOGS At Work Limited is solely responsible for the delivery of the Program. FOGS ARTIE Academy staff are not employed by the DoE.

Section 3: Parent/Guardian Consent

By signing this form (below) I/we acknowledge that:

  1. I/we are the parent(s) or legal guardian(s) of the child named

  2. I/we have read Section 2 of this form and understand what the Program involves and that FOGS At Work Limited is solely responsible for delivery of the Program;

  3. I/we have read and understood Section 1 of this form and give permission for the Department of Education to disclose the personal information listed in Section 1 to FOGS At Work Limited and its staff for the purposes of my/our child's application to join, and participation in, the Program.

  4. I/we have read and understood Section 1 of this form and give permission for FOGS At Work Limited t to disclose the personal information listed in Section 1 to the Department of Education and its staff for the purposes of my/our child's participation in the Program.
